You can self-certify your illness for the first 7 days of absence.

Many employers have their own Self Certification Forms. If your employer doesn’t have there own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.

If your illness is for more than 7 days you will need to request a 'Fitness to Work' certificate.

If you have not seen or spoken to a doctor about your illness you will need to book a telephone appointment to discuss this request. If you have recently been discharged from hospital or have already been seen by the doctor you can request a certificate via the e consult system.

If you have already been receiving extension certificates from the doctor you can submit your request via the e consult system, you may not need to be seen or you may be contacted to arrange a follow up appointment.

Please allow 48/72 hours before collecting you certificate from the surgery.
