PATIENT INFORMATIOM ABOUT USING KLINIK
PATIENTS WHO HAVE SUBMITTED A KLINIK FORM TO THE SURGERY PLEASE DO NOT PHONE THE SURGERY ASWELL
All patients will need to complete a consultation form to contact the surgery, whether online, by phone or in person at the surgery only one form to be submitted.
This will allow us to direct patients to the most appropriate clinician at the most appropriate time and, hopefully, reduce the stress of trying to get through on the phone.
The form can be used for most enquiries including requesting an appointment, test results, medication queries, sick notes and requesting letters.
House visit requests should still be made by phoning the surgery.
If you have internet access we ask that you complete the Klinik form yourself via computer, laptop, tablet or mobile. This will free up the phone lines for those patients who are not able to use the online service.
If you phone or attend the surgery the receptionist will go through the same questions and your request will be processed in the same way as those sumbitted online.

The healthcare team at Caldicot Medical Group are committed to providing high quality care to our local population. We are a multidisciplinary team of GPs, nurses, pharmacists and physiotherapists as well as administrative support staff who ensure the smooth running of the practice.

Introducing Klinik
From June 2022 we are launching a new system to help manage access to appointments and other services at the surgery. The new system is called Klinik.
All patient will need to complete a consultation form to contact the sugery whether online, by phone or in person at a surgery. This will allow us to direct patients to the most appropriate clinician at the most appropriate time and, hopefully, reduce the stress of trying to get through on the phone at 8.00 am.
