When attending the practice:

  • Please wear a face covering.  This is mandatory under Welsh government guidelines and applies to anyone over the age of 11 unless there is a medical exemption. You can find instructions for making your own face covering here.

  • Please attend your appointment on your own if possible.

  • Please use hand sanitiser when entering and leaving the surgery.

  • Please observe 2m social distancing.

New Patient Registration

Registration at our surgery is a simple process. Please bring proof of identity and address; we would be grateful if you could make an appointment for a basic health check. This will enable us to assess your health needs and update your records. Please bring a list of your regular medication and a sample of urine to the appointment.

Children, 16 and under, do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

Disabled Patient Facilities

Disabled patient facilities are also available at this practice (three car parking spaces, ramp, full disabled access at front door, disabled toilets etc.).
